My participant is generally successful at following directions and respecting authority and peers. My participant can occasionally struggle with following directions and respecting authority and peers but is generally responsive to corrections. My participant tends to need extra encouragement to participate due to being shy or anxious. I understand I may be asked to stay on premise if it helps my child feel more comfortable participating or if they request to sit out of an activity. My participant has a history of struggling with following directions and respecting authority and peers. I understand I may be asked to stay on premise to help my child be more successful in overcoming challenges. I also understand my child may be asked to no longer attend if coaches feel behavior is disruptive or harmful to the overall environment.

Do you agree or not agree to the Minor Waiver/RELEASE OF LIABILITY FOR MINOR PARTICIPANTS: I AGREE THAT IN CONSIDERATION OF MY CHILD/WARD, BEING ALLOWED TO PARTICIPATE IN ANY WAY IN THE 518 CHRISTIAN YOUTH ATHLETIC LEAGUE AND RELATED EVENTS AND ACTIVITIES THAT 1. The risk of injury to my child/ward from the activities involved in these programs is significant, including the potential for permanent disability and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and, 2. I, for myself, children, family, spouse, pet, and participant/child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, knowingly and freely assume all such risks, known and unknown, even if arising from the negligence of the releasees or others and assume full responsibility for my child/ward’s participation; and, 3. I willingly agree to comply with the program’s stated and customary terms and conditions for participation. If I observe any unusual significant concern in my child/ward’s readiness for participation and/or in the program itself, I will remove my child/ward from the participation; and, 4. I, for myself, children, family, spouse, pet, and participant/child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby release and hold harmless 518 Christian Youth Athletic League, it’s coaches, directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, owners and lessors of premises used to conduct the event (“Releasees”), with respect to any and all injury, disability, death, and/or loss or damage to person or property incident to my child/ward’s involvement or participation in these programs, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law; and, 5. I, for myself, children, family, spouse, pet, and participant/child/ward, and on behalf of my/our heirs, assigns, personal representatives and next of kin, hereby indemnify and hold harmless 518 Christian Youth Athletic League, it’s coaches, directors, officers, officials, agents, employees, volunteers, other participants, sponsoring agencies, sponsors, advertisers, owners and lessors of premises used to conduct the event (“Releasees”) from any and all liabilities incident to my child’s/ward involvement or participation in these programs, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law; and, 6. I hereby give consent on behalf of participant/child/ward, as applicable, to have 518 Christian Youth Athletic League, it’s coaches, directors, officers, officials, agents, employees, volunteers, emergency medical technician, physician, nurse or other healthcare professional provide medical assistance and/or treatment and agree to be financially responsible for the cost of assistance and/or treatment. I hereby authorized transportation to a healthcare facility should it be considered warranted. I acknowledge and understand that certain risks or injury, including but not limited to, concussions, serious bodily injury, or death are inherent in playing sports and may result from the players actions, the actions or inactions of others, or a combination of both. In signing below, I certify the player received all necessary medical clearances to participate fully in the 518 Christian Youth Athletic League, without restriction or condition. To the maximum extent permitted by law, I hereby agree to release, waive, hold harmless, and indemnify 518 Christian Youth Athletic League, it’s coaches, directors, officers, officials, agents, employees, volunteers, and other participants, against any claim by or on behalf of the participant/child/ward named above, as a result of their participation in the 518 Christian Youth Athletic League and related events and activities. We will have printed waivers for you to sign at drop off. By submitting this form digitally you HAVE GIVEN UP SUBSTANTIAL RIGHTS AND AGREE TO IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
